Rapid Response Discharge
With the Rapid Response Discharge Program Discharge Planners can call or email a referral and within hours we will respond.
HERE IS WHAT YOU CAN EXPECT FROM US
- Complete client assessment within 4 hours
- Caregiver match within 4 hours of the client's signing of the contract
- Immediate start of care within 24 hours of signing the contract.
Home Care Referral
Do you have patient discharging home and they could use some additional help to maintain their independence. Submit a referral and one of our intake coordinators will reach out to schedule an assessment.
Hospital Discharge
Have a client/patient discharging home in a couple days and they need additional assistance send us the referral and we can assist with their needs
